The City of Collinsville is in the process of preparing a Multi-Hazard Mitigation Plan. This will be a strategic planning guide to reduce the community's risk from man-made and natural hazards, in fulfillment of the Hazard Mitigation Grant Program requirements of the Federal Emergency Management Agency. This survey is intended to gain an understanding of the public's awareness and concern of hazards that could impact the City of Collinsville.

For each of the following hazards, please circle the corresponding number indicating your level of concern for that hazard affecting Collinsville.
