1. Event Day is
Saturday, June 12, 2010. 2. ALL display material is the responsibility
of the vendor. 3. Vendor assumes ALL responsibility for loss, theft,
or damage. 4. Vendor can unload and set up from 6:00 a.m. until 8:00 a.m.
on Saturday, June 12, 2010. Vendor's vehicles must be out of the event area
by 8:00 a.m. 5. Booth space is 10' x 10' at $30.00 per booth space. Limit
of 2 booths per vendor. Vendor will be responsible for all booth structures
and furnishings. NOTE: Tables and chairs will NOT be provided. 6. No
exclusive rights for the sale of any item will be guaranteed. 7.
Vendor location will be determined at time of check-in on the day of the event.
8. All sales during the event are subject to local and state sales tax of 9.267%.
If you have a sales tax
ID, we MUST have a copy for our files. If you do not have a tax ID number,
a form will be provided to you at the time of check-in. Sales tax will be
collected by Collinsville Downtown, Inc. 9. Vendor may begin tear down at
4:30 p.m. and your area must be clean and vacated by 6:00 p.m. 10. This is
an outdoor event and there are no guarantees on the weather. Registration fees
are not refunded unless the event is cancelled by Collinsville Downtown,
Inc. 11. PLEASE
NOTE: City Guidelines state that at least 4' (feet) of sidewalk thoroughfare
must be available for foot traffic. Sidewalk vendors must set up to allow for
proper passage. 12.
Application and all applicable fees must be received by June 1, 2009.
HOLD HARMLESS
CLAUSE: "The exhibitor/vendor assumes the entire responsibility and liability
for losses, damages and claims arising out of injury to persons or damage to displays,
equipment, or other property brought upon the premises and agrees to indemnify,
defend and hold harmless the Collinsville Main Street Program and its owners,
servants, agents and any employees against all claims or expenses such as losses,
including reasonable attorney's fees, arising out of the use of space at this
event. The exhibitor/vendor understands that the Collinsville Main Street Program
does not maintain insurance covering the exhibitor/vendor property or lost revenues
and it is the sold responsibility of the exhibitor/vendor to obtain such insurance. Signature
