1. Event day is Saturday, June 13, 2009 in Collinsville, Oklahoma. Set-up
by the Vendor will be no than 8:00 a.m. on the morning of the event, unless other
arrangements have been discussed and approved. Exhibitor's vehicles must be out
of the event area by 8:00 a.m. 2. All sales during the event are subject to
local and State sales tax of 8.17%. The tax monies will be collected at the end
of the by Collinsville Downtown, Inc. An Oklahoma Sales Tax number must be included
with this application. We must have a copy of your tax ID for our files. A sales
tax form will be provided to every vendor at time of check-in, even to those vendors
without a sales tax number. 3. Your location will be determined upon your
arrival on the day of the event. 4. If electric is requested, the Vendor will
be responsible for bringing their own extension cords. Please note: Extension
cords or tables will NOT be provided. 5. Food Vendors: Must provide a
current copy of your business license, insurance and health permit. The Tulsa
County Health Department will be present that day and they will be checking. They
will issue a temporary health permit if you do not have one. 6. You may begin
tear down after 4:30 p.m. and your area must be clean and vacated by 6:00 p.m.
7. This is an outdoor event and there are no guarantees on the weather. Registration
fees are not refunded unless Collinsville Downtown, Inc. cancels the event.
8. PLEASE NOTE: City Guidelines state that at least four feet (4') of sidewalk
thoroughfare must be available for foot traffic. Sidewalk vendors must set up
to allow for proper passage. 9.
We cannot guarantee that any vendor will have an exclusive on any item. Application
and all applicable fees must be received by June 1, 2009.
HOLD
HARMLESS CLAUSE: "The exhibitor/vendor assumes the entire responsibility
and liability for losses, damages, and claims arising out of injury to persons
or damage to displays, equipment or other property brought upon the premises and
agrees to indemnify, defend and hold harmless the Collinsville Main Street Program
and its owners, servants, agents, and any employees against all claims or expenses
for such losses, including reasonable attorneys fees, arising out of the use of
space at the festival. The exhibitor/vendor understands that the Collinsville
Main Street Program does not maintain insurance covering the exhibitor/vendor's
property or lost revenues and it is the sole responsibility of the exhibitor/vendor
to obtain such insurance. Signature
