5. Consideration and possible action relating to a request for Commission approval of the Consent Agenda. All matters listed under “Consent” are considered by the City Commission to be routine and will be enacted by one motion. Any Commissioner may, however, remove an item from the Consent Agenda by request. A motion to adopt the Consent Agenda is non-debatable.

A. Approval of November # 3 Payroll in the amount of $47,921.04.
B. Approval of Claims in the amount of $18,246.08.
C. Approval of Minutes from November 19, 2012 Regular Meeting.
D. Approval of Cemetery Deeds: Fred Tobin – 2 spaces; Larry Stephens – 1 space; Steve Forrest – 2 spaces.
E. Approval of agreement between the City of Collinsville and Tulsa County regarding 12th Street overlay labor.

6. Discussion and possible action concerning items removed from the Consent Agenda. -- n/a

7. Discussion and possible action to accept bid from Vista Construction. in the amount of $75,600 on Collinsville American Legion Roof Project. -- Approved, best of the 2 bids received. Will include concrete block formed beams and reinforcement/replacement of damaged top rows of bricks. (Previous Coverage -- July 2012)

8. Discussion and possible action to appoint Adam Steidley to the Park Board to fill the unexpired term of Pam Webb which will expire May 7, 2016. -- Approved

9. Discussion and possible action for approval of a one-time, lump sum salary increase for personnel. -- Approved (already budgeted "Christmas stipend" $300 full-time and $150 part-time)

10. Discussion and possible action to proceed with asphalt overlay on the city street running between South 19th Street and South 21st Street. Un-named street is located south of West Main Street and North of the Garden Apartments. Cost not to exceed $12,000. -- Approved, hope to apply soon before weather turns bad.

11. Discussion and possible action to approve Ordinance Number 865, an Ordinance amending the employee Retirement System, defined contribution plan for the position of City Manager for the City of Collinsville and Collinsville Municipal Authority, Oklahoma; providing retirement benefits for eligible employees of City of Collinsville and Collinsville Municipal Authority, Oklahoma; Pertaining to plan design; providing for employer pickup of required contributions; providing for repealer and severability; and declaring an emergency. -- Tabled (the emergency portion would have required an additional commissioner. )

12. Consider a motion declaring an emergency on Ordinance Number 865 for preservation of public health, peace and/or safety making it effective immediately. -- Tabled

13. 15. City Manager’s Report. -- Pam Polk reported on these topics:
* City Employee's Christmas Party is Dec 13th at Main Street Event Center
* Christmas Parade is Dec. 8th
* Community Angel Tree is being handled by Dividing Bread
--- City will be sponsoring a family of 4 directly
* Wade Lenhart hired for billing position (from Electric Dept)
* Jemery Plowman switched from part-time to full-time
* Cliff Jones new city street superintendent
* Top 10 finalist at Crown Opry Friday Dec. 7th
* Likely S-Curve ribbon cutting soon
* Grant for American Legion roof unlikely

6. Public Works Director Report. -- Phil Stowell reported on these topics:
* Progress on city water drainage study (including recommendations for some pipe size upgrades likely)
* 2 inch asphalt overlay at apartments off 19th soon
* 12th Street asphalt overlay (about 1/2 mile) likely in spring
* Charlie's Park - two 6ft picnic tables (from corrections dept) expected by end of week. Expect multi-purpose shelter later (parent shade / bus stop for kids).
* Park trail progressing
* Pam Polk noted that Phil's dad was terminally ill (in hospice at Grove)
